Subcutaneous emphysema, pneumothorax and pneumomediastinum are well-known complications of invasive ventilation in patients with acute hypoxemic respiratory failure. We determined the incidences of air leaks that were visible on available chest images in a cohort of critically ill patients with acute hypoxemic respiratory failure due to coronavirus disease of 2019 (COVID-19) in a single-center cohort in the Netherlands. A total of 712 chest images from 154 patients were re-evaluated by a multidisciplinary team of independent assessors; there was a median of three (2–5) chest radiographs and a median of one (1–2) chest CT scans per patient. The incidences of subcutaneous emphysema, pneumothoraxes and pneumomediastinum present in 13 patients (8.4%) were 4.5%, 4.5%, and 3.9%. The median first day of the presence of an air leak was 18 (2–21) days after arrival in the ICU and 18 (9–22)days after the start of invasive ventilation. We conclude that the incidence of air leaks was high in this cohort of COVID-19 patients, but it was fairly comparable with what was previously reported in patients with acute hypoxemic respiratory failure in the pre-COVID-19 era.
Two key air pollutants that affect asthma are ozone and particle pollution. Studies show a direct relationship between the number of deaths and hospitalizations for asthma and increases of particulate matter in the air, including dust, soot, fly ash, diesel exhaust particles, smoke, and sulfate aerosols. Cars are found to be a primary contributor to this problem. However, patient awareness of the link is limited. This chapter begins with a general discussion of vehicular dependency or ‘car culture’, and then focuses on the discussion of the effects of air pollution on asthma in the Netherlands. I argue that international organizations and patient organizations have not tended to put pressure on air-control, pollution-control or environmental standards agencies, or the actual polluters. While changes in air quality and the release of greenhouse gases are tied to practices like the massive corporate support for the ongoing use of motor vehicles and the increased prominence of ‘car culture’ globally, patient organizations seem more focused on treating the symptoms rather than addressing the ultimate causes of the disease. Consequently, I argue that to fully address the issue of asthma the international health organizations as well as national health ministries, patient organizations, and the general public must recognize the direct link between vehicular dependency and asthma. The chapter concludes with a recommendation for raising environmental health awareness by explicitly linking the vehicular dependency to the state of poor respiratory health. In this article a generic fault detection and diagnosis (FDD) method for demand controlled ventilation (DCV) systems is presented. By automated fault detection both indoor air quality (IAQ) and energy performance are strongly increased. This method is derived from a reference architecture based on a network with 3 generic types of faults (component, control and model faults) and 4 generic types of symptoms (balance, energy performance, operational state and additional symptoms). This 4S3F architecture, originally set up for energy performance diagnosis of thermal energy plants is applied on the control of IAQ by variable air volume (VAV) systems. The proposed method, using diagnosis Bayesian networks (DBNs), overcomes problems encountered in current FDD methods for VAV systems, problems which inhibits in practice their wide application. Unambiguous fault diagnosis stays difficult, most methods are very system specific, and finally, methods are implemented at a very late stage, while an implementation during the design of the HVAC system and its control is needed. The IAQ 4S3F method, which solves these problems, is demonstrated for a common VAV system with demand controlled ventilation in an office with the use of a whole year hourly historic Building Management System (BMS) data and showed it applicability successfully. This project researches risk perceptions about data, technology, and digital transformation in society and how to build trust between organisations and users to ensure sustainable data ecologies. The aim is to understand the user role in a tech-driven environment and her perception of the resulting relationships with organisations that offer data-driven services/products. The discourse on digital transformation is productive but does not truly address the user’s attitudes and awareness (Kitchin 2014). Companies are not aware enough of the potential accidents and resulting loss of trust that undermine data ecologies and, consequently, forfeit their beneficial potential. Facebook’s Cambridge Analytica-situation, for instance, led to 42% of US adults deleting their accounts and the company losing billions. Social, political, and economic interactions are increasingly digitalised, which comes with hands-on benefits but also challenges privacy, individual well-being and a fair society. User awareness of organisational practices is of heightened importance, as vulnerabilities for users equal vulnerabilities for data ecologies. Without transparency and a new “social contract” for a digital society, problems are inevitable. Recurring scandals about data leaks and biased algorithms are just two examples that illustrate the urgency of this research. Properly informing users about an organisation’s data policies makes a crucial difference (Accenture 2018) and for them to develop sustainable business models, organisations need to understand what users expect and how to communicate with them. This research project tackles this issue head-on. First, a deeper understanding of users’ risk perception is needed to formulate concrete policy recommendations aiming to educate and build trust. Second, insights about users’ perceptions will inform guidelines. Through empirical research on framing in the data discourse, user types, and trends in organisational practice, the project develops concrete advice - for users and practitioners alike - on building sustainable relationships in a resilient digital society.
