Introduction: Fall rates and fall-related injuries among community-dwelling older adults (≥65 years) are expected to increase rapidly, due to the aging population worldwide. Fall prevention programs (FPPs), consisting of strength and balance exercises, have been proven effective in reducing fall rates among older adults. However, these FPPs have not reached their full potential as most programs are under-enrolled. Therefore, this study aims to identify promising strategies that promote participation in FPPs among community-dwelling older adults. Methods: This is an exploratory qualitative study. Previously, barriers and facilitators for participation in FPPs by older adults had been identified. Next, six strategies had been designed using the Intervention Mapping approach: (1) reframing; (2) informing about benefits; (3) raising awareness of risks; (4) involving social environment; (5) offering tailored intervention; (6) arranging practicalities. Strategies were validated during semi-structured interviews with communitydwelling older adults (n = 12) at risk of falling. Interviews were audio-recorded, transcribed, and analyzed following a qualitative thematic methodology, with a hybrid approach. Results: All strategies were considered important by at least some of the respondents. However, two strategies stood out: (1) reframing ‘aging’ and ‘fall prevention’: respondents preferred to be approached differently, taking a ‘life course’ perspective about falls, and avoiding confronting words; and (2) ‘informing about benefits’ (e.g., ‘living independently for longer’); which was mentioned to improve the understanding of the relevance of participating in FPPs. Other strategies were considered important to take into account too, but opinions varied more strongly. Discussion: This study provides insight into potential strategies to stimulate older adults to participate in FPPs. Results suggest that reframing ‘aging’ and ‘fall prevention’ may facilitate the dialogue about fall prevention, by communicating differently about the topic, for example ‘staying fit and healthy’, while focusing on the benefits of participating in FPPs. Gaining insight into the strategies’ effectiveness and working mechanisms is an area for future research. This could lead to practical recommendations and help professionals to enhance older adults’ participation in FPPs. Currently, the strategies are further developed to be applied and evaluated for effectiveness in multiple field labs in a central Dutch region (Utrecht).
Introduction: Falls and fall-related injuries in community-dwelling older adults are a growing global health concern. Despite effective exercise-based fall prevention programs (FPPs), low enrollment rates persist due to negative connotations associated with falls and aging. This study aimed to investigate whether positive framing in communication leads to a higher intention to participate in an FPP among community-dwelling older adults. Methods: We conducted a twosequence randomized crossover study. We designed two flyers, a standard flyer containing standard terminology regarding FPPs for older adults, and a reframed flyer highlighting fitness and activity by reframing ‘fall prevention’ as an ‘exercise program’ and ‘old’ as ‘over 65 years’. With a Mann– Whitney U test, we investigated group differences regarding the intention to participate between the flyers. A sensitivity analysis and subgroup analyses were performed. We conducted qualitative thematic analysis on open-ended answers to gain a deeper understanding of participants’ intention to participate. Results: In total, we included 133 participants. Findings indicated a significantly higher intention to participate in the reframed flyer (median = 4; interquartile range = 1–6) compared to the standard flyer (median = 2; interquartile range = 1–4) (p = 0.038). Participants favored more general terms such as ‘over 65 years’ over ‘older adults’. Older adults who were female, not at high fall risk, perceived themselves as not at fall risk, and maintained a positive attitude to aging showed greater receptivity to positively-framed communications in the reframed flyer. Additionally, already being engaged in physical activities and a lack of practical information about the FPP appeared to discourage participation intentions. Discussion: The results in favor of the reframed flyer provide practical insights for designing and implementing effective (mass-)media campaigns on both (inter)national and local levels, as well as for interacting with this population on an individual basis. Aging-related terminology in promotional materials hinders engagement, underscoring the need for more positive messaging and leaving out terms such as ‘older’. Tailored positively framed messages and involving diverse older adults in message development are essential for promoting participation in FPPs across various population subgroups to promote participation in FPPs among community-dwelling older adults.
Background: The need for effective continuing education is especially high in in-hospital geriatric care, as older patients have a higher risk of complications, such as falls. It is important that nurses are able to prevent them. However, it remains unknown which interventions change the behavior of nurses. Therefore, the aim of this study is to identify intervention options to change the behavior of hospital nurses regarding fall prevention among older hospitalized patients. Methods: This study used a mixed method design. The Behavior Change Wheel (BCW) was used to identify intervention functions and policy categories to change the behavior of nurses regarding fall prevention. This study followed the eight steps of the BCW and two methods of data collection were used: five focus groups and three Delphi rounds. The focus groups were held with hospital nurses (n = 26). Geriatric experts (n = 11), managers (n = 13) and educators (n = 13) were included in the Delphi rounds. All data were collected within ten tertiary teaching hospitals in the Netherlands. All participants were included based on predefined in- and exclusion criteria and availability. Results: In Geriatric experts’ opinions interventions targeting behavior change of nurses regarding fall prevention should aim at ‘after-care’, ‘estimating fall risk’ and ‘providing information’. However, in nurses’ opinions it should target; ‘providing information’, ‘fall prevention’ and ‘multifactorial fall risk assessment’. Nurses experience a diversity of limitations relating to capability, opportunity and motivation to prevent fall incidents among older patients. Based on these limitations educational experts identified three intervention functions: Incentivisation, modelling and enablement. Managers selected the following policy categories; communication/marketing, regulation and environmental/social planning. Conclusions: The results of this study show there is a discrepancy in opinions of nurses, geriatric experts, managers and educators. Further insight in the role and collaboration of managers, educators and nurses is necessary for the development of education programs strengthening change at the workplace that enable excellence in nursing practice. Het hoofddoel van het FRIEND-project is om tot succesvolle implementatie van integrale valpreventie-interventies in de wijk te komen. Aan de hand van actiegericht onderzoek streven we naar inzicht en realisatie van succesvolle implementatiestrategieën, waarbij specifiek wordt ingegaan op interprofessionele samenwerking en op het werven en behouden van ouderen voor deze valpreventie-interventies (VPI’s). Dit is belangrijk omdat de potentie van bewezen effectieve VPI’s niet wordt bereikt doordat deze in bepekte mate daadwerkelijk worden geïmplementeerd. Door een betere implementatie in de dagelijkse praktijk kunnen VPI’s een belangrijke bijdrage leveren aan het behoud van (fysieke) zelfredzaamheid van thuiswonende ouderen. De huidige implementatie van bestaande effectieve VPI’s is niet optimaal door o.a. onvoldoende aansluiting bij de wensen en behoeften van ouderen en onvoldoende samenwerking tussen de professionals in de wijk. Het nu ook niet goed kunnen betrekken van deze stakeholders, zou ten koste gaan van effectiviteit van de door ons ontwikkelde implementatiestrategieën. Extra tijdsinvestering is daarom noodzakelijk om ouderen en professionals te bereiken en te motiveren, daarnaast om activiteiten zo vorm te geven dat ze passen bij de wensen en behoeften van betrokkenen t.a.v. de Corona-maatregelen en de actuele omstandigheden. Het uitgangspunt blijft om zoveel mogelijk vast te houden aan het oorspronkelijke onderzoeksplan. Echter, hebben sommige onderzoeksactiviteiten meer tijd gekost en zal dat de komende periode ook zo zijn doordat meer moeite gedaan moet worden voor werving, er extra maatregelen worden genomen bij fysieke ontmoetingen, er meerdere scenario’s uitgedacht worden en er mogelijk meerdere sessies georganiseerd moeten worden. Daarnaast zullen we door de opgedane vertraging later dan oorspronkelijk gepland, starten met de echte implementatie in de wijk. De budget-neutrale verlenging die is geboden zal worden ingezet om de monitoring zo dicht mogelijk bij de oorspronkelijk periode van 48 maanden te laten zijn. De op te leveren resultaten zijn onveranderd t.o.v. het oorspronkelijke voorstel.
INXCES will use and enhance innovative 3D terrain analysis and visualization technology coupled with state-of-the-art satellite remote sensing to develop cost-effective risk assessment tools for urban flooding, aquifer recharge, ground stability and subsidence. INXCES will develop quick scan tools that will help decision makers and other actors to improve the understanding of urban and peri-urban terrains and identify options for cost effective implementation of water management solutions that reduce the negative impacts of extreme events, maximize beneficial uses of rainwater and stormwater for small to intermediate events and provide long-term resilience in light of future climate changes. The INXCES approach optimizes the multiple benefits of urban ecosystems, thereby stimulating widespread implementation of nature-based solutions on the urban catchment scale.INXCES will develop new innovative technological methods for risk assessment and mitigation of extreme hydroclimatic events and optimization of urban water-dependent ecosystem services at the catchment level, for a spectrum of rainfall events. It is widely acknowledged that extreme events such as floods and droughts are an increasing challenge, particularly in urban areas. The frequency and intensity of floods and droughts pose challenges for economic and social development, negatively affecting the quality of life of urban populations. Prevention and mitigation of the consequences of hydroclimatic extreme events are dependent on the time scale. Floods are typically a consequence of intense rainfall events with short duration.
